I've read it through once. There's a fair bit of 'speed kills' phraseology, but table 2 seems to contain the real deal - 'poor observation' is top of the tree for all groups except emergency service vehicles. And what's 'distance to stop judgement'? I haven't found a defintion, but it sounds as if we're taking about some aspect of 'safe speed behaviour' and the numbers suggest we're very good at it.
The accident description in figure 1 could possibly indicate miscoding - it depends how carefully they examined the information. Another primary explanation might be that the truck driver wasn't attentive to the road ahead. We'd need to know the visibility distance to be confident.
