But surely what usually happens is that drivers see something too late, rather than not at all, and cannot lose enough speed to avoid the collision.

If your scenario above was commonplace, then it would be a strong argument for lower, strictly-enforced speed limits to mitigate the effect of unavoidable collisions.

I wasn't by any means suggesting that that's a commonplace scenario - but I have seen it happen on numerous occasions. I just included it to counter the usual implication that slowing down gives you more time to react etc.
