Sorry chaps. Just don't know. I've tried to find the original notification but am failing to find it.
It worked for me and after putting it up I tested it and it did go to the survey. So odd. If I can find it I will of course add up a new link.

The most requested follow up survey subject by far concerned the motorway speed limit. Please take the time to complete the survey on this and other hot motorway related topics.
